white bread vs. wheat bread

    Why can't hemo dialysis pt's have wheat bread but are allowed white bread? It seems to me like wheat is best for you anyway and white has a lot of added sugar. Please explain???

    As far as I was always told and understood, the whole wheat/white bread issue is based in dietary sources of phosphorus. I was told to avoid wheat bread/whole grain products while I was on dialysis because they are a lot higher in phosphorus in comparison with the plain old white bread. I think that is one of the main reasons behind that....can't reallly remember if there was anything else behind that reasoning, anyone else know other reasons for it?

      Actually, I believe it has to do with levels of potassium....if it were phosphorous, I think we could increase the binders. But with potassium, we don't have binders. Yes, it does seem odd as wheat is obviously healthier and has more fiber than white....so much of what we learn on dialysis, is 'unlearning!'.....goes against the healthier habits of diet!

        white bread and whole wheat bread

        white bread - based on 1 slice
        Protein - 2.5gm
        Sodium - 144mg
        Potassium - 30mg
        Phosphorus - 27mg
        KCAL - 77

        whole wheat bread - based on 1 slice
        Protein - 3gm
        Sodium - 149mg
        Potassium - 77mg
        Phosphorus - 65mg
        KCAL - 69

        I hope this helps. I love whole wheat bread better, every once in awhile I eat white bread.

          If you think about it, most bread - even white bread - is 'wheat' bread. As a prior poster answered, the issue comes from "whole wheat, whole grain" breads. The less processed the bread, the more phosphorus remains. So in general, white breads are allowed, and whole grains are not so much.

          The binder comes in to help bind excess phosphorus taken in from all foods with phosphorus - which is most every food having some because phoshporus is so plentiful in the natural food supply (it's part of the body's energy cycle, so it's a good thing for phosphorus to be so readily available -- until your kidneys don't work right).

          Phosphorus allowance in general is limited to 800-1000 mg / day for dialysis patients, and that's not really easy to do, so the 'high phosphorus' foods are limited/restricted, and binders are used to help control the rest taken in from being absorbed into the blood stream.

          Keep learning all you can -- following this meal plan is one of the hardest parts of being on dialysis, and we here at DaVita are trying to help you have access to everything you need to learn all you can to stay healthy!

              Binders are a medicine that binds the phosphorus so it won't enter your bloodstream.

                If you are gluten intolerant, then go with white bread.

                  My husband is Stage 4 and has trouble with his potassium. He is only allowed white or up to 60% whole wheat; no bananas, potatoes (unless leached and only occasionally) and no tomatoes. There are a few other things higher in potassium, but actually there are lots of foods to choose from. I thought we were all suppose to have high fiber goods, but our specialist told us it was better to eat the white than have a heart attack - which you can't argue with.
